SPECIFIC GYNECOLOGICAL EXAMINATIONS IN MILAN

COLPOSCOPY

WHAT IT'S FOR 

Colposcopy is a Level II examination that highlights benign, pre-neoplastic, and neoplastic lesions of the lower genital tract (cervix, vagina, vulva). The main indications are: abnormalities found at Pap-Test (ASCUS, AGC, ASC-H, L-SIL, H-SIL), cervical polyps, cervical ectopia, vaginal and vulvar lesions, condylomata, HPV test positivity, and abnormal genital bleeding.



WHEN IT IS PERFORMED

Colposcopy is an outpatient, painless, noninvasive examination. Its duration is just 10 to 15 minutes. The best time for its performance is about 10 days after the end of menstruation. No special preparation is required before the examination. In the previous 24 hours, sexual intercourse, insertion of tampons, eggs, candles etc. should be avoided. Anesthesia is not required. The presence of a chaperone is not required. It can be performed during pregnancy, with no risk to the fetus.



HOW IT IS PERFORMED

The examination consists of viewing the external genitalia (vulvoscopy), vagina (vaginoscopy) and cervix (colposcopy) with progressively magnifying lenses. It visualizes lesions that would not be visualized with the naked eye after applying solutions for a few minutes to the mucous membranes of the genital organs that may cause mild burning. A biopsy that causes a little bleeding may, in addition, be performed. In other cases, the lesions can be removed immediately by electrocautery (LLETZ). After the examination, normal daily activities can be resumed

WHAT IT'S FOR

The HPV Viral Test detects the presence of Papilloma Virus in the human body. 


From the onset of sexual activity, women are exposed to several risk factors for the development of cervical cancer. Human Papilloma Virus (HPV) infection, a sexually transmitted virus, is the most important risk factor. In particular, histotypes 16, 18, 31, 33, 35, 39, 45, 51, 52, 56, 58, 59, 68, defined as high-risk, increase the risk of cervical cancer occurrence. Whereas, histotypes 6, 11, 42, 43, 44, defined as low risk, can cause warts on the external genitalia. It is not recommended for women under the age of 25, as the virus is widespread in the young population. 


WHEN IT IS PERFORMED

Generally, the Test is prescribed by the gynecologist in the presence of an abnormal Pap-Test result. No prior preparation is necessary. It is helpful to avoid intercourse, vaginal douches, vaginal medications, spermicides or lubricants in the two days before the exam. It is best not to perform the examination on menstrual days. After the examination, normal daily activities can be resumed.



HOW IT IS PERFORMED

The Test is simple, painless and quick. It takes only a few minutes. It is performed by the gynecologist in an outpatient setting. The patient lies on the couch in the gynecological position. The gynecologist inserts the speculum (a small retractor) into the vagina to visualize the upper part of the vagina and the cervix (neck, portio) of the uterus. At this point he performs the sampling with a soft brush (cytobrush).



RESULTS

Negative outcome means that the patient has not been infected with any type of HPV virus that causes cervical cancer, while Positive outcome means that the patient has been infected with the high-risk type of HPV virus (histotype 16, 18, 31, 33, 33, 35, 39, 45, 51, 52, 56, 58, 59, 68), which can cause cervical cancer. This does not mean that cancer is already present, but only that one of the risk factors for its development is present. At the same time, its presence may be only transient and healing be spontaneous within a few years. In case of positivity, follow-up of the patient is necessary. Depending on clinical cases, other tests may be prescribed: colposcopy with biopsy.
